How to fill it in
- Replace the bracketed attributes, category and price with your own; keep the scales unchanged between campaigns.
- Send it after the recipient has had time to use the product, not on delivery day. How long depends on the product.
- Ask permission before using any answer or media publicly (question 17 and the upload task) and record the answer with the response.
- Read the open answers to questions 9 and 16 before the averages; they usually explain the scores.
